Step 2: Fill out the form and obtain any necessary authorizations!

Please obtain all necessary authorizations for the patient's visit, as not having it will delay the consultation/procedure scheduling. In order to see a specialist, one must have a referral from another physician (usually the primary care physician) and the necessary insurance authorization for the visit.

Step 3: Fax Form To Gold Coast (860-444-0823)

Please send all information with the referral form (recent H&P, insurance information, medication list, and copies of sleep studies or other pertinant testing). After recieving the fax, we will enter the patients information into our computer system and call the patient to schedule an appointment as soon as possible.
