In order to become a patient at Gold
Coast, ask your primary care
physican to provide a referral to be
seen at our office. Our easy to
use referral can be downloaded from our website
here.
Physicians may download our
referral form, obtain the necessary
authorizations,
and fax the paperwork to

Pulmonary Disease. It is a
lung disease in which the lung is
